About us
Wheatfields Hospice is in Headingley, Leeds. We provide expert palliative care. We support people living with a life-limiting condition, and also help their loved ones and family.
In palliative care no day is the same. Working on our inpatient unit provides a unique opportunity because we care for patients with such a variety of conditions and symptoms. About the Role
Every day, you’ll have the opportunity to work alongside our Sue Ryder Nurses, occupational therapists, physiotherapists, complementary therapists, family support team, spiritual support and palliative care consultants, learning new skills and sharing and gaining knowledge.
The successful applicants will play an active role within our team. It's all about giving our patients the best possible quality of life. The support we offer is tailored to meet individuals' medical, social, spiritual and emotional needs.
Key responsibilities include but are not limited to:
• Provide and coordinate compassionate, safe, effective clinical and therapeutic care to a caseload of patients as delegated by the Registered Nurse/regulated practitioner
• Recognise changes in patients’ conditions which may require the intervention of others and ensure that timely referrals are made or escalation procedures are followed
• Collect, collate, evaluate and report information, maintaining accurate patient records in all formats, including written and electronic patient information systems
• Accurately undertake risk assessments using approved documents and assessment tools. Use agreed patient protocols, standard operating procedures clinical guidelines which reference actions in accordance with the outcome of risk assessment
• Act as an advocate for patients, ensuring that any barriers to patients’ opinions and wishes are challenged and patients’ dignity maintained
About you
You will hold a Nursing Associate qualification, be eligible to work in the UK and must be registered as Nurse Associate with the NMC.
We are open to discussions regarding long and short shifts. All of our ward staff are required to work internal rotation covering nights, weekends and bank holidays.
While Palliative care experience is desirable, we will accept applications from nurse associates of varying backgrounds with a desire to learn. You will need to be able to demonstrate the ability to work well within a team, use initiative and have excellent communication skills.
