We are calling on Registered Nurses (Band 6) to join our rapidly expanding, specialist Hospice at Home Team at Sue Ryder Thorpe Hall Hospice.
Our Hospice at Home service supports palliative patients and their families to stay at home. Our expert team provide nursing care, advice on managing symptoms as well as emotional support.
The role involves assessing, reassessing and prescribing care for patients who require end of life/palliative care and crisis support. Assessment will be both face to face and via telephone. You will also be responsible for supporting the team of senior nursing assistants out in the community and also coordinating in the absence of the Team Leader.
What you need to join our team:
• Registered Nurse, preferably with experience in palliative care and community nursing (other areas will be considered)
• Have excellent communication and great interpersonal skills
• Organised and able to prioritise your work load
• A knowledge of systmone would be an advantage
You will have background and skills in:
• Nurse-led management of long term conditions/palliative care
• Leading clinical practice development for client group
• An ability to communicate difficult messages to service users and their families
• Negotiation and conflict management skills
• An ability to provide teaching and mentorship in a clinical setting
• Able to effectively manage change
As this role is primarily based in the community, you are required to have your own transport (mileage paid) and a full drivers licence.
The shifts are 12 hours 8am – 8pm over 7 days per week. We are happy to discuss full time or part time hours and also have bank positions available.
