Leckhampton Court Hospice is one of several palliative care providers in Gloucestershire, providing practical end of life nursing care, in our In Patient Unit, in a patient’s home and we are now opening our community services. Leckhampton Court Hospice is the only inpatient unit for specialist end of life care in Gloucestershire and the home of our new, developing community service. We help people live the best life they possibly can.
About the Role,
As an experienced practitioner, the post holder will be expected to create an environment of continuous quality improvement, developing clinical practice and ensuring that delivery of care is regularly reviewed embedding the CQC agenda. This new and exciting role is to grow our community services to facilitate patients attending the hospice to access a variety of services; including a social drop in, medical outpatients, art classes, exercise classes, fatigue/breathlessness management, clinical/social support, carer support, family support and much more to include complimentary therapies in the future. The post holder will be responsible for the running of the community services and the overall coordination of all activities that take place in the Community Services Building. Responsible for managing referral pathways and ensuring the provision of service is efficient and responsive within contracted timeframes: -
• To effectively manage the community services resources working from Leckhampton Court Hospice and to ensure the provision of co-ordinated and comprehensive services to patients
• The post holder will work collaboratively across Health and Social Care boundaries within Gloucestershire, identifying opportunities and driving a culture of local integration with all key stakeholders, which puts the patient at the centre of service delivery. This could include some presentations to third parties and commissioners.
• Ensure the provision of specialist palliative care interventions for those individuals referred to the services
• Act as a resource to, and advise and support all those involved in the care and management of the client group
• This is a new role with opportunities to develop a fledgling service for the benefit of all
About you,
Applicants must:
• Hold a full UK driving licence and access to a car, as some community visits may be required
• Understand that the role may require some weekend and Bank Holiday shifts
• Accept inclusion to the wider hospice on call rota
We are looking for someone who possesses the following qualifications/qualities:
• Registered Nurse or Registered AHP
• Relevant degree / demonstrable experience preferably to Masters Level
• Mentors and Assessors award or equivalent (Desirable)
• Evidence of continued professional development
• Evidence of collaborative working in a multi-professional environment
• Up to date clinical experience
• Experience of managing a team
• Experience of managing resources according to service user need
• An ability to challenge the traditional boundaries using an evidence base
• Experience of managing a process of change and supporting people through that change
• An awareness of the needs of service users from minority groups
• Effective communication of complex information in a variety of settings
• Excellent interpersonal and team building skills
• Negotiation with agencies on highly complex issues regarding contracting arrangements.
• Developed communication skills for delivering key messages to a range of stakeholders both internal and external some at a very senior level.
• Strong leadership experience including development planning and budget management.
