Sue Ryder has an exciting opportunity for a Clinical Team Manager to join the dedicated team at Sue Ryder Lancashire Neurological Care Centre. As an experienced practitioner, the post holder will be responsible to create an environment of continuous quality improvement, developing clinical practice and ensuring that delivery of care. You will be responsible for the overall running of the teams, managing referral pathways and ensuring the provision of service is efficient and responsive within contracted timeframes.

About the role
•Responsible to assess, plan, implement and evaluate individualised care using evidence based guidelines. To identify gaps in service and developing solutions to address these. To involve patients, carers and families in the development of care and support their needs
•To take responsibility for reviewing care plans and the change of treatment where needed and communicating decisions to all involved in the care of the individual. To take responsibility for informing carers of specific aspects of care e.g. complications of disability / drug efficacy and side effects
•Responsibility to act in an advisory role and to take responsibility for the assessment of all aspects of care for the service users in the community, identifying opportunities for growth and improvement, regularly reporting activities and outcomes internally and to external commissioners
•To ensure that clinical audit activity is completed and learning incorporated into practice. To support the Head of Clinical Services to deliver the quality improvement plan
•Responsible to provide line management support to the team including appraisal, performance management and training and development, undertake clinical and management supervision. To manage the performance of community teams and services.
•To maintain own professional development and to co-ordinate and ensure the continuing professional development of the community team. To identify training needs and work with Head of Clinical Services and Education Lead to ensure appropriate development of clinical team members. Work with the Practice Educator to actively contribute to the development and delivery of unit teaching and mentorship programmes
•To ensure that record keeping is of the highest standards and meets all statutory and professional requirements; both written documentation and electronic databases
•Facilitating the safe and smooth running of the Service, the post holder is expected to maintain a safe working environment, maintain service user confidentiality, and occasionally take on reasonable additional tasks
About you
•Registered Nurse
•Relevant degree in Healthcare or equivalent knowledge
•Management qualification or equivalent experience
•Evidence of continued professional development
•Proven experience of leading clinical teams and successful change management
•Evidence of collaborative working in a multi-professional environment
•Substantial and up to date clinical experience
•Experience of managing resources according to need
•An ability to challenge the traditional boundaries using an evidence base
•Experience of managing a process of change and supporting people through that change
•An awareness of the needs of service users from minority groups
•Management experience at NHS band 7 or equivalent
•Working knowledge of Care Inspectorate standards relevant to care setting
•Able to prioritise workload and manage competing priorities
•Able to work independently and with others to ensure achievement of results
•Able to analyse data and write reports
•Current full driving license
