About the role:
This is an excellent opportunity for an experienced nurse to join our busy but very friendly community team at St John’s Hospice. If you are an enthusiastic and committed Senior registered nurse who is interested in a rewarding career within palliative care we'd like to hear from you!

The role encompasses a 24 hour telephone support line, working alongside Nurse’s or Health Care Assistant’s. You will provide palliative care support and advice to our Patients on the Palliative Care Hub register, their families and other care professionals whilst signposting to other agencies as and when required. We will require excellent assessment, planning, referral, implementation and evaluation skills to ensure our patients receive high standards of individualised care. You will also be required to support and lead some of the HCAs and be responsible for the allocation of the team’s visits.

About You:
• NMC registered, with post-registration experience in palliative, end of life care.
• Ability to be flexible, positive and work within the wider team.
• Excellent soft communication skills.
• Ability to lead and empower others.
• Live our values of Supportive, Impactful and Connected and practice our attached behaviours.
• A knowledge of SystemOne would be advantageous
• A knowledge of the independent sector
