We are looking for a dynamic, forward thinking and experienced family support practitioner who is passionate about specialist palliative care to join our Family Support Team at The Duchess of Kent Hospice.
As a Family Support Practitioner you will provide level 3 (NICE Guidance 2004) psychological services for patients and their families which address pre-bereavement and post bereavement needs. You will complete a psycho-social assessment for patients and their family/carer including children to identify social strengths, needs, and goals. You will work in close partnership with the clinical teams within the hospice and other organisations.
The Duchess of Kent is a Sue Ryder hospice, based in Berkshire and providing care across Reading, Wokingham and Newbury. We're here for people aged 18 and over dying from life limiting conditions like cancer, heart failure and lung disease. We offer a huge range of support, from inpatient care in our 15-bed hospice, set on the edge of Reading next to beautiful parkland, to Day Services and community care which helps people stay at home longer. Our expert care team includes doctors, nurses, care assistants, physiotherapists, occupational and complementary therapists, social workers, bereavement support workers, volunteer befrienders and spiritual care workers.
The successful candidate will have evidence of post qualifying counselling experience in a health or social care service. They will have experience of multidisciplinary working and an understanding of end of life issues.
Essential requirements for the post:
Experience of counselling in a healthcare setting
Experience of teaching and training staff and volunteers
Creating and facilitating carers/bereavement groups.
Registration with the regulatory bodies is mandatory: BACP / UKCP
