About us
Sue Ryder Palliative Care Hub South Oxfordshire operates from Battle Barns in Crowmarsh, Wallingford. We support people aged 18 and over who are living with life-limiting conditions such as cancer, heart failure and lung disease.
Our team of Sue Ryder Nurses, doctors, physiotherapists, occupational therapists, family support and befrienders combine specialist medical care for managing pain and other symptoms, with emotional, practical and spiritual support. We provide a Hospice at Home service, day hospice, and a care coordination and advice line.

About the Role

Sue Ryder are pleased to invite applications from experienced nurses to join a well-established
friendly team as a Clinical Nurse Specialist in Wallingford. You will be supporting patients with an End of Life diagnosis to remain in their homes. This role predominately a telephone triage role.

The duties of this role will include assessing referrals for admission to the service, performing comprehensive assessment of patients, Planning, implementing and evaluating care delivery according to changing healthcare needs, identifying issues which are deemed to be actual/potential palliative care emergencies, compiling action plans to mitigate distress, Ensuring such plans are developed, documented and communicated in a collaborative way with other professionals, patients and their families. As well as:

• Using advanced assessment skills to identify reversible symptoms, develop a plan of care and implement
• Providing holistic specialist care for patients in accordance with their needs, wishes and culture. This includes clinical assessment, planning care, monitoring progress, requesting appropriate investigations, and acting on the results
• Establish and maintain effective communication, often of a highly complex and contentious nature, with patients, carers/relatives, and professionals across health and social services. Using verbal and written communication and negotiating skills breaking down barriers to understanding with various individuals

About you

We are looking to recruit a registered nurse with experience as a community palliative care nurse or would consider a nurse with experience in palliative care and evidence of continued professional development in this speciality. You should have excellent communication skills and qualifications in advanced clinical assessment, mentorship and non-medical prescribing or be prepared to undertake training.
