Sue Ryder provides incredible hospice and we care for people facing a life-changing or terminal diagnosis. We do whatever we can to act as a safety net or our patients and their loved ones during the most difficult time of their lives.
We’re now seeking a Registered Nurse to join us in and provide amazing services to patients in their own homes.
The South Oxfordshire community service provided by Sue Ryder offers community services, including Hospice at Home for people aged 18 and over who are living with conditions like cancer, heart failure and lung disease. Working with the Hospice at Home team is our Clinical Nurse Specialist team.
Your Role:
As a Registered Nurse, you will provide high quality nursing care to end of life patients in their own home, supervising and supporting Nurse Assistants.
On an ongoing basis, you will assess, plan, implement and evaluate care for patients and escalate complex issues to the Clinical Nurse Specialist or Medical Consultant for their advanced assessment or a case review.
Your role will also include:
- Leading a team of Senior Nursing Assistants in the provision of care.
- Providing high standards of holistic, individualised care and support to patients and families, through a process of assessment, planning, implementation and evaluation.
- Acting as a key worker in the community.
- Being responsive to patients needs and flexibly visit according to suggested triaged response time.
- Working shifts, triaging calls to the centre and visiting patients as necessary, often as a lone worker.
- Ensuring patient records are accurately written, in line with Hospice regulations.
- Working with the Clinical Nurse Manager to plan and implement change and development within the Hospice where this would increase effectiveness and efficiency of practice, deputising for them as required.
- Providing telephone support to patients, families and other professionals.
- Providing supervision to Nursing Assistants.
- Maintaining and promoting effective and efficient working and communication with other community professionals, hospital professionals, voluntary, private and statutory agencies and other hospices.
- Participating in the audit process and encourage colleagues to embrace the opportunities gained from audit.
- Assisting in the identification and meeting of educational needs of Assistant nurses.
- Ensuring end of life procedures are carried out sensitively and appropriately.
- Verifying death when it is appropriate to do so in line with hospice policy.
- Demonstrating effective communication with key workers.
- Participating in clinical supervision
- Liaising with professionals from other agencies for the benefit of the patient and their family.
- Communicating sensitively and effectively both verbally and in writing with patients, families and colleagues including volunteers.
About You:
To be considered as a Registered Nurse, you will need:
- Relevant post-registration training or be willing and able to gain this within 1 year
- To be an NMC Registered Nurse, preferably with experience in neurological or specialist palliative care
- The ability to demonstrate a commitment to continued professional development
- To have completed an NMC recognised accredited mentor training and the ability to mentor students and less experienced staff
Other organisations may call this role Staff Nurse, RGN, Registered General Nurse, Nurse, Nurse Practitioner, Palliative Care Nurse, or Senior Nurse.
